Participation Waiver
CH³ uses one general participation agreement for adult participants across its programs and activities. The current agreement is intended to cover 12 months from the date it is signed, subject to the terms written in the agreement.
Who Should Use This Form?
This agreement is for participants 18 years of age or older. A separate parent or legal guardian agreement is required for participants under 18. If you are completing paperwork for a keiki participant, please contact CH³.

How It Works
- Read the agreement on your phone or download the PDF.
- Print it, enter your name, and sign and date it.
- Bring the completed agreement to your next CH³ session.
No printer? CH³ will have paper copies available at the session.
Once CH³ has a current signed agreement on file, you generally will not need to complete another agreement for every CH³ session during its 12-month coverage period. A new or additional form may be required if CH³ updates the agreement or if a particular activity, school, facility, partner, insurer, or applicable law requires one.
Paper signature notice
This website does not collect online signatures or completed waiver forms. Please bring your signed paper agreement to a CH³ session. Do not email or upload a completed agreement unless CH³ specifically asks you to.
